To apply the patch you need the program PPF-O-Matic available here :
http://www.romhacking.net/utilities/356/

As shown in the screenshot on that page, the program is very straightforward, in the interface you select the iso of your game, you select the PPF patch and click Apply, that is all.

Make sure to back up the iso first in case you want to go back to how it was before for some reason (or that you make a mistake and apply the wrong patch).
