3) It's VPS slowdown, FPS stays at 30 at all times
4) Doesn't happen in small areas but happens in all larger areas (village, non-camp areas)

GPU: GTX 560 TI
RAM: 16GB
OS: Windows 7

7) The speed goes back to 60 and runs perfectly if I swap back to windowed mode.

Do you have some sort of overlay program like MSI afterburner that can show you your CPU/GPU usage percentages in fullscreen? Maybe your CPU or GPU are underclocking in fullscreen for some reason?

This is the best I could do. Couldn't get something to run on top of fullscreen PPSSPP, as soon as I alt-tabbed to the resource monitor it would speed up again. You can see the lag spike though:
