Got first time PFEAR on a real PSOne when it fell down from me and some stuff got detached from the board. Got second time PFEAR when I got a debug model of PSOne and tried to boot stuff that it's not built to without some modification of some sort. Got PFEAR third time on ePSXe when I failed to dump bios properly the first time I tried myself. I'm also pretty possitive PS2 and Gamecube have weird error audios on hardware failure errors, I encountered them both but not on my hardwares.
