I removed the dedicated server from the test environment and used one of the clients to host the server, disabled all the firewalls, AFK'd a 50 minute quest, and it appears that the issue has disappeared. It should be noted that I only ran the test a single time (finding something else to do for 50 minutes can be surprisingly hard sometimes).
I opened these ports on the dedicated server and again AFK'd a 50 minute quest (the client firewalls are still disabled, so I didn't bother messing with those), and everything seems to work. Here, I should mention that I only ran the test twice.
In any case, I now just have to see how well this works when I actually try to play with my buddies. Thanks guys.

UDP: 10000, 20001-20004
Those appeared as listening ports in "Resource Monitor". 10000 is used inside GH and 20001-20004 inside quests.
